TEA Form - Get this form from your child's previous/current School Attendance

Glasses or Contacts.

Parent must sign application and have notarized. Notary available if testing
with Rowlett Driving School.

All forms will be filled out during first day of class.

The Department of Public Safety will ONLY accept ORIGINAL DOCUMENTS, no copied
or facsimiles. These documents will be returned to you after application has
been entered into the DPS system and printed.
